Max, though I have 2-3 active credit card, I made it a point to only use one card at any one time. The other 2 are just for back-up, in case I need to cancel the one that I'm not happy with.

The form is made in such a way that you just need to print the form as it is, and then fill in the blanks. The details are all the credit card companies need to proceed with the cancellation.
Hehe...sorry, I can come up with the list of fax numbers, but updating the numbers to ensure its relevancy is going to be too much trouble. I'll leave that part to the cardholders ;-)
